Your employer chose Zenith because we’re workers’ compensation insurance specialists. Our highest priority is to get injured employees the medical care they need to help them recover and return to work as soon as possible. If you’re injured performing your job, be sure to tell your employer immediately so you can receive the care you need.

To help you recover, we’ve identified doctors experienced in treating work-related injuries. And since an insurance claim can be confusing, it’s our job to answer your questions and help you throughout the process. Our claims and medical employees will discuss how your injury occurred and how to find a physician. They’ll also explain what benefits and rights you have. Our goal is to make the process easier so you can focus on healing.

Your employer is responsible for reporting your workers’ compensation claim to Zenith. However, if your employer has not submitted your claim, you may contact us directly. You can submit a claim at any time. Be sure to have the following information ready:

  1. Your employer’s business name and policy number (if available)
  2. How you were injured and the date and time of injury
  3. Your name, address, Social Security number, date of hire, occupation, wages, and date of birth
  4. The name, address, and phone number of your medical provider if you’ve already received medical attention for your injury

Once we receive your claim, we can also answer any questions you have about workers’ compensation processes and benefits.

Getting proper medical care is critical to your recovery. Under your employer’s workers’ compensation policy, Zenith pays for all necessary medical treatment for your accepted workplace injuries. Our knowledgeable medical staff is available to work with your physician and evaluate your treatment plan. A claims specialist or one of our medical management nurses will explain your benefits.

California Only – Medical Access Assistants are specially trained customer service staff who can help you find a physician in the Zenith Medical Provider Network (ZMPN), get an appointment with your physician, and answer other network access questions. To request help, call Zenith and choose Medical Access Assistant from the options provided:

If you receive a prescription from your doctor at any time for your injury, we encourage you to use a participating pharmacy to have it filled. We offer a pharmacy benefit management program administered by Cadence Rx.

When you have an open and accepted claim with Zenith, we pay for all your approved prescriptions. You’ll receive a welcome packet from Cadence Rx with a pharmacy benefit card. Please present this card at a participating pharmacy each time you drop off and pick up a prescription for your work-related injury. Be sure to inform the pharmacist that you have a workers’ compensation claim with Zenith Insurance Company.

California Only – Pharmacy benefits for California injured workers are provided through the Zenith Pharmacy Network and you are required to obtain your medications from a participating pharmacy. Please ensure you only use the pharmacy card for work-related injuries, as trying to obtain other medications may be considered fraud.

Should your injury prevent you from working, your benefits may cover a portion of your lost wages. We’ll keep your employer informed of your progress, so they’re ready to welcome you back when your doctor says you’re able to return to work. Benefits vary widely by state. Your claims specialist will determine if you are eligible for this benefit and will explain all aspects of your benefits and the payment process.
