As a result of California SB 899, signed by Governor Schwarzenegger, Zenith has established the Zenith Medical Provider Network (ZMPN) - a customized network of physicians, hospitals, outpatient facilities and ancillary services which covers the entire State of California. The network was effective February 1, 2005 for any workers' compensation claims.

NOTE: California policyholders are required to provide a copy of our ZMPN Brochure to their employees immediately following a workplace injury. The brochure contains a "Medical Authorization" form, which the employer is required to complete and provide to the treating physician prior to medical treatment.
