 | Experience and relationships go a long way in effectively treating workplace injuries and managing claim costs.
If one of your employees has a workplace injury or illness, Zenith provides treatment through our Zenith Medical Provider Network (ZMPN)—a customized network of physicians, hospitals, outpatient facilities and ancillary service providers throughout California. |

Employers are required by law to distribute medical network notices to all employees.
Notice: Effective Oct. 8, 2010, the State of California has instituted new medical network notice regulations for California businesses. Zenith will be mailing all California policyholders updated copies of the California Posting Notice (DWC 7)/Medical Provider Network Notice (one for each work location) to help ensure that our policyholders are in compliance with the new regulations, as non-compliance can potentially result in a $7,000 civil penalty from the state. The mailing will also include samples of the new DWC 1 form, Facts about Workers' Compensation brochures, and the new ZMPN Implementation and New Employee Notice.
Please reference the chart for specific instructions on which materials you are required to distribute at policy inception, time of hire and time of injury. These materials explain workers’ compensation rights and benefits, as well as the requirement to treat with a medical network provider if a workplace injury occurs.
For your convenience, we offer pre-assembled “Time of Hire” and “Time of Injury” packets. You can order free copies of these materials through Zenith’s Resource Library. The materials are listed under the “Claim & Policy Resources—California” category. Create an account now—It's quick and easy!
To complete your policy inception requirement, order a copy of the ZMPN brochure for each of your employees using Zenith’s Resource Library. Distribution of the brochure should be accomplished in the manner most suitable for your organization. Please ensure that your method of distribution is designed to reach all employees. For example, if all employees receive a physical paycheck, you can include the brochures with the paycheck—make sure to document the distribution date. Then, if an employee is injured, you can prove that the ZMPN brochure was provided by showing records that the employee cashed their paycheck. If you choose an alternate distribution method, please document the distribution method (in detail) and the date the brochures were distributed.

Employers must also complete and post the California Employee Notice.
The State requires that you keep the “Notice to Employees: Injuries Caused by Work (DWC-7)” posted at all times in a conspicuous location frequented by employees; the notice must be displayed in all work locations in both English and Spanish. The State requires that you insert the appropriate information in the blank spaces provided and that the information is kept up-to-date. To locate a ZMPN provider to list on the poster, use the provider search tool at the top of this page or call our customer service team at 1-800-440-5020.
