State Posting Notice

Employee Notice

Before a workplace injury occurs, you should communicate your choice of medical provider to your employees.  For assistance creating a panel of medical providers in your local area, please contact Zenith's customer service team at 800-440-5020. (Note: This is not required by law, but can be helpful in reducing claim costs).
