- Notify Zenith online. Complete our online change of address notification form and we'll take care of the rest.
- Notify your insurance agent via email. Your insurance agent can then relay your contact change request to Zenith.
- Notify Zenith on your premium payment coupon. Simply write in your updated contact information on the front of your payment coupon when you mail in your premium check.
- Notify Zenith via fax. Complete our change of address notification form and fax it to the number listed below for your state.
