Agency Forms:

Electronic Commission Authorization Form

Policyholder Forms:

Electronic Funds Transfer (EFT) Authorization Form
Policyholders can use this form to sign up for EFT payments for their premium invoices (there is no limit on the amounts an insured can pay). Once signed up, EFT payments will continue for the current policy and future renewals until the insured notifies Zenith in writing to discontinue EFT. There is no additional charge for this service.
