If you experience a workplace injury or illness, Zenith provides treatment through our Zenith Medical Provider Network (ZMPN)—a customized network of physicians, hospitals, outpatient facilities and ancillary service providers throughout California.

Unless you predesignated a personal physician using an appropriate form before this injury occurred, you are required to treat with a ZMPN doctor.

Please review the following materials, which are sent to all California employees after an injury is reported:

• DWC 1: CA Workers' Compensation Claim form:  This form should be completed and provided to your employer.  
• ZMPN “Time of Injury” brochure: This brochure includes information about Zenith’s California medical network.
