You can report by phone 24 hours a day, 7 days a week. We will help arrange medical treatment and answer your questions about workers' compensation benefits.

Alternatively, if you would like to report your workers' compensation claim by email, you may send an email to our claims team. Please be sure to provide all the necessary information (listed below).

For life-threatening medical emergencies, call 911 to arrange transportation to the nearest medical facility.

OSHA requires that employers report certain serious work-related injuries or illnesses within 8 hours of the injury or the employer's knowledge of the injury. While Zenith is not a party to that process, we are offering this reminder of the potential requirement and encourage you to check your state reporting rules.


When you call or email Zenith, please have the following information ready:

  1. Your policy number;
  2. Description, date and time of accident; and
  3. The injured employee's name, address, Social Security number, date of hire, occupation, wages and date of birth.

Note: If the employee received medical attention for the injury prior to your call, we need the name, address and phone number of the provider.
