ZMPN Provider Manual:  In addition to contact and billing information, this manual provides our Zenith medical providers with reference materials regarding statutory and regulatory requirements as set forth in the California labor code.

ZMPN Brochure:  Provides information to employees concerning the network, including Zenith’s policies on Continuity of Care and Transfer of Care.

ZMPN Provider Exclusion Appeal Policy: This policy sets forth the appeal processes and procedures that are available to providers when a provider is excluded from the California ZMPN. The policy applies to all network exclusions regardless of whether the decision to exclude the provider was made at the time of initial application and credentialing or after the provider had participated in the ZMPN for a period of time.
