If one of your employees suffers a workplace injury or illness, Zenith provides treatment through our Zenith Medical Provider Network (ZMPN)—a customized network of physicians, hospitals, outpatient facilities and ancillary service providers throughout California. In addition, Zenith provides coverage for prescription medications for compensable work-related injuries through the Zenith Pharmacy Network (ZPN).
Unless your employees predesignate a personal physician using an appropriate form before an injury occurs, they are required to treat with a ZMPN doctor.
Time of Injury Requirements: As of May 1, 2013, please give the Zenith Employee Notice (which replaces the Time of Injury brochure) as well as the DWC-1 and release of records forms listed below (known as the Time of Injury kit). The new Zenith Employee Notice contains the modified Zenith Medical Provider Network (ZMPN) as a result of SB863 and the modified Zenith Pharmacy Network (ZPN) notice.
