These materials present information concerning Zenith’s practices and procedures. Not every policy or procedure utilized by Zenith has been included.

California

How to Join the Zenith Network: Contact Zenith Provider Relations team at 1-800-440-5020 if you are interested in a possible network appointment.

Find a ZMPN Provider: Use our online search to find medical specialists and facilities in your geographic area. If you would like to make a referral for an injured worker, please contact the claims examiner for authorization.

Network Notice: Zenith Insurance Company and ZNAT Insurance Company maintain the Zenith Medical Provider Network (ZMPN) for treatment of injured workers in California. Zenith pays network contracted rates for services provided to injured workers when the services are provided by a ZMPN provider. Network providers should assure that all treatment for injured workers is provided through the ZMPN. This includes referrals to specialists and ancillary providers.

Zenith utilizes the RxAmerica pharmacy benefits program for fulfillment of prescriptions. Injured workers are provided information on pharmacies participating in the program and can fill prescriptions at the network pharmacy of their choice.

Provider Records: The accuracy of provider data is important. If your individual or medical group TIN, service address, billing address, affiliations or other information has changed, please provide the corrected or new information to Zenith’s Provider Relations Department as soon as possible. This will assist in maintaining the accuracy of our provider records.

ZMPN Provider Manual: In addition to contact and billing information, this manual provides our valued Zenith medical providers with reference materials regarding statutory and regulatory requirements as set forth in the California labor code.

ZMPN Brochure: Provides information to employees concerning the network, including Zenith’s policies on Continuity of Care and Transfer of Care.


Texas

Network Notice: Zenith Insurance Company utilizes the Zenith Healthcare Network (ZHCN) contracted through Interplan and as appropriate the Coventry network for treatment of injured workers in Texas. Zenith pays network contracted rates for services provided to injured workers when the services are provided by a network provider. Network providers should assure that all treatment for injured workers is provided through the appropriate network. This includes referrals to specialists and ancillary providers.

Provider Records: The accuracy of provider data is important. If your individual or medical group TIN, service address, billing address, affiliations or other information has changed, please provide the corrected or new information to Zenith’s Provider Relations Department as soon as possible. Network Notice: Zenith Insurance Company utilizes network arrangements to provide treatment for injured workers in many states. Zenith pays network contracted rates for services provided to injured workers when the services are provided by a network provider. Unless state law provides otherwise, network providers are required to provide all treatment for injured workers through the applicable network. This includes referrals to specialists and ancillary providers.

Zenith also utilizes pharmacy benefits programs for fulfillment of prescriptions in many states. When a pharmacy benefits program is utilized, the injured workers are provided information on pharmacies participating in the program and can fill prescriptions at the network pharmacy of their choice.
