Agency Forms:
Electronic Commission Authorization Form
Use this form to sign up for automatic deposits for your agency commission payments. For more information or questions regarding automatic deposits, please contact us at 1-800-440-5020 or PremiumAccountingCommissions@thezenith.com
Policyholder Forms:
Electronic Funds Transfer (EFT) Authorization Form
Policyholders can use this form to sign up for EFT payments for their premium invoices (there is no limit on the amounts an insured can pay). Once signed up, EFT payments will continue for the current policy and future renewals until the Insured notifies Zenith in writing to discontinue EFT. There is no additional charge for this service.
Premium Payment Electronic Debit Authorization Form
This is an alternate form insureds can use to make a payment on their policy. A fax machine is required. There is no limit as to how much an insured can pay and there is no additional charge for this service.
Policyholder Request for Address Change
Policyholders should complete this form with their most up-to-date contact information and return it to Zenith.
